LOCATION FOR TREATMENT:

(Florida Resident) As per Florida Law, the client must be in the state of Florida at the time of service. 

​The client must be in a confidential location for HIPPA.

COSTS RELATED TO SERVICES:

The client will be responsible for all treatment costs associated with the patient’s care which will include but are not limited to deductibles, out-of-pocket co-insurance, session fees, or no-show/late cancellation fees. Therapy is $125.00 per session. No show/late cancellation/late reschedule fee $60.00.

REFUND/RETURN POLICY:

​The client will receive a full refund for service if they cancel/reschedule service 24 hrs before service time. If the client cancels or reschedules 24 hrs before service, they are subject to a full refund. If the client fails to cancel or reschedule 24 hrs before service, they are subject to a partial refund. No show/late cancellation/late reschedule fee $60.00.

The client cannot receive a refund or return for services rendered.
